Office Policies:

     * Family Members:  Please refrain from discussing medical issues of the members who are not the subject of that day's visit. This ensures Privacy and avoids making decisions without the benefit of the member's Medical Record.

     * Fasting cholesterol tests: you are required to fast for 12-14 hours. You may have water and
medication(hold diabetic meds until after your meal). Prior to your fasting, please follow a low fat diet for
48 hours, to allow your body to clear excess triglycerides.

    * Walkins will be accommodated, but priority will be given to scheduled visits.

    * Office hours are not rigid. Closing hours are determined by that day's schedule.

    * Reporting results: All lab and xray results are carefully reviewed by the Doctor. Abnormalities
of an urgent nature will be reported to the patient immediately by the doctor. Other abnormal results
will be reported to the patient in a timely manner. It is physically impossible to verbally report all
normal blood tests and xrays. However, we are happy to give them to you verbally, by fax or mail
when you request them. Lab results of long term nature are best discussed in the office upon
follow up visits.

    * No shows: a fee will be assessed for all no show visits.  This fee will be added to future visits.
A 24 hour cancellation notice is expected.

    * Telephone prescriptions :  At the doctor's discretion, some phone prescriptions will be issued.
In general, this is not done for new and less well established patients. It is not encouraged for
antibiotics and pain killers.

  * Annual or Routine Physicals:  Please check to see if your plan pays for well ( routine ) visits. A well visit cannot be filed as an Illness visit without the presence of Major Chronic disease such as Diabetes, Hypertension etc.
